Sorry it took me a little longer to respond. We've been quite busy lately doing all the paper work and getting things sorted out. It is great to speak to someone who's in Canada as well. I can give you the heads up on the medical questionnaire. I had obtained it through work. They have them for us as I work in a Call Center for eBay. As such, if you're unable to perform specific tasks of your position, it is required to be filled out to excuse you from them. I had mine filled out as I just did not feel well doing phones. I found it stressful at times and I didn't want to cause any stress at all to the babies.
Other than that, for mat leave, you just need to have your EI forms filled out or any maternity forms through your work and your request for maternity leave written up. I had an additional paper to fill out (Short Term Disability) as I will be taking an early leave due to having twins. They suggested I fill out the Short Term leave papers as it will pay me more than just being on EI. We don't know that I'll be approved yet, but if not, I always have EI to fall back on.
I presume this is your second baby? I had a look at a few pictures. Do you know if it's a girl or a boy? Congrats! It's so exciting. This is my first pregnancy and I wound up with two little boys, can't wait. Definitely nervous but I think my fiance and I will do well handling it.

I am only 9 weeks but I do not fit in any of my jean or work pants any more, and I do not fit on maternity pants yet because they are too big! So yes, it is a problem. I find myself wearing sweatpants at all times.... I fell a lot better with sweatpants. Well keep us posted!!! Congrants!!!
